1. CLIENT AGREEMENT

To be completed by the parent

Please review and sign the CLIENT AGREEMENT

2. EXPECTATIONS FOR COLLEGE COUNSELING

To be completed by the student & parent

Student and parent must read and sign this AGREEMENT OF EXPECTATIONS

3. STUDENT IN-TAKE FORM

To be completed by the student.

Please complete the STUDENT IN-TAKE FORM before our initial meeting.

In addition, please forward copies of the student's transcript and test scores to info@thecollegenavigators.com

Here is a copy of the SERVICE AGREEMENT

Once all documents are signed/received, an initial meeting will be scheduled.

The initial meeting will establish a timeline for the meetings allotted per package. The initial meeting does not count toward meetings/hours.
